Professionally removed from our recently purchased 2011 Honda Element LX when we had it replaced with a new receiver/back-up camera. It was in perfect working order at time of removal and has been in a box since. I have not removed it from the brackets to check model number; however, it matches pictures from OEM Used Auto Parts site, comes with the plastic compartment labeled "HONDA", and is marked "Made in Japan."
