Provide information on services and programs to older and/or disabled adults and family caregivers; assist functionally impaired older and disabled adults maintain their maximum level of independence; link older and disabled adults with community resources; assist older adults and disabled to obtain services and benefits; facilitate referrals to Long Term Care Services and Supports (LTSS).

Outreach/Information and Referral
 Promote awareness of the various options that are available in the community, especially among underserved, hard-to-reach, and private paying populations; as well as options individuals can use to plan ahead for their long-term needs
 Provide information individually as requested or in group settings
 Conduct person-centered interviews with callers to determine the need for options counseling, access assistance and/or referral to additional services and supports
 Provide appropriate assistance over the telephone, through electronic communications, or in person
 Link individuals with needed services and supports - both public and private through appropriate referrals to other agencies, organizations, or businesses, per the individual's request and consent
 Follow up to see if services were put in place or if more assistance is needed
 Conduct outreach in accordance with the ADRN's outreach and marketing plan
Options Counseling
 Conduct person-centered interviews by phone, in the office and/or home visit to understand individuals' circumstances, needs, preferences, and current formal or informal services and supports
 Assist individuals to identify desired and available options (including personal, public, and private services and supports)
 Facilitate a decision-support process (weighing pros/cons of various options)
 Assist, as requested, to develop an action step plan or long term service and support plan that is directed by the individual
 If requested, provide person-centered one-on-one supportive counseling and assistance to ensure successful linkage to services and supports; or arrange for services
 If requested, provide directly, or refer to established partner, for the following: assistance with applications for public services, employment assistance, benefits counseling, futures planning, mobility assistance, and accessing participant directed services when available
 As requested, advocate on behalf of individuals to help them access services
 When assistance is provided, follow up to see if services and/or supports were established or if more assistance is needed and requested
 Participate in the ADRN's secure client management information system to track and improve options counseling outcomes
Other requirements
 Develops knowledge of eligibility process and requirements for public LTSS programs offered in the state/local area such as Medicare, Medicaid, Older Americans Act, Veterans services, state funded aging and disability programs
 Understands philosophy and importance of person centeredness, self direction and independent living and the importance of each person directing the planning process
 Understands the role of family, caregivers, and community as potential important informal and formal supports
 Understands basic issues related to different populations, including older adults, persons with physical disabilities, persons with intellectual/developmental disabilities and persons with Alzheimer's disease or cognitive impairment
 Understands how culture and diversity may impact communication, health knowledge, and health behaviors
 Understands reporting requirements for neglect or abuse
 Expected to become certified in Option Counseling during first year of employment
 Expected to become Alliance of Information and Referral Systems (AIRS) Certified during the second year of employment
 BA in human services or related degree and two years of experience providing direct human services or Master's degree in behavioral or health sciences and one year of paid on-the-job social service experience (preferred)
 Demonstrated ability working with seniors or people with disabilities. Preferred experience would include a general knowledge of the aging process and at least two years of full-time paid or volunteer experience working with the elderly or people with disabilities
 Demonstrated ability to work with diverse ethnic populations
 Ability to work independently
 Ability to problem solve and work with stressful situations
 Demonstrated ability in oral and written communication
 Demonstrated ability in computer programs such as Microsoft Office and utilization of databases

Work is performed in an office environment, during client home visits and other assignments outside of the office. The office environment may include a share workspace and may be fast paced with frequent interruptions. Potential hazards may include working with emotionally unstable, non-compliant or aggressive clients and complex family dynamics; significant local driving; second hand tobacco smoke; aggressive animals and exposure to contagious diseases.
